SALES AGREEMENT

30 DAY TRIAL PERIOD AND MONEY BACK GUARANTEE (Florida Statute 468.1245).  Under Florida law you, as a person buying a hearing aid(s), have the right to cancel this purchase within 30 days of receiving the hearing aid(s).  If the device is lost during the first 30 days, it will be replaced per the warranty restrictions, but cannot be returned for credit. If the hearing aid(s) must be repaired, remade, or adjusted within the 30-day trial period, the trial period will stop until the hearing aid(s) is returned to you or until 4 days after you are notified that the hearing aid(s) is ready to be reclaimed, whichever is earlier.  In the event that you decide to return the hearing aid(s) within the 30-day trial period, you have the right to receive a refund of the total purchase price, upon the return of the hearing aid; in good working condition, less a charge not to exceed $350 for one hearing aid and $700 for two hearing aids to cover costs for shipping expense. It is your right to seek out medical clearance before purchasing a hearing device and by signing this agreement you are waiving the need for medical clearance. LOST HEARING AIDS CANNNOT BE RETURNED

The charges on this purchase agreement are for the hearing aid(s), earmold(s), dispensing and fitting services along with adjustments, supplies, receivers, batteries, and other parts of the hearing aid(s) FOR 1 Calendar year from Date of Purchase *Shipping Handled by customer.

By purchasing from our online store you are assuming regular care and maintenance.

The following is our Care and Maintenance recommendation:

□ Cleaning aid with alcohol prep pads or cloth that is provided. Please do this every night before inserting into the charger-EVERY NIGHT BEFORE PUTTING IN CHARGER 

□ Changing Wax Filters (CERUSTOP)- every 2 months to both devices- DEPENDS ON WAX PRODUCTION
